A screening process that includes a criminal records check, reference checks and an interview are required to participate in the Child Advancement Project. Training requirements must also be completed. I hereby grant permission to THRIVE to conduct this screening process. I authorize all law enforcement agencies and references to provide necessary and relevant information about me to THRIVE and I release them from any and all liability as a result of sharing that information.
I understand that in order to be matched as a mentor in the Child Advancement Project, I must successfully complete each phase of the screening and training process. I understand that misrepresentation or omission of facts requested is cause for non-placement. I also understand that although I successfully complete the screening and training process that I might not be placed. I release THRIVE from any and all liability associated with this process or their decision to select me as a participant in this program.
If placed I agree to fulfill the mentor responsibilities to the best of my ability.
For security purposes, your Social Security number will be required in order to process all necessary background checks. You will be asked to provide this information at your preliminary interview.
Due to the importance of the work performed by CAP mentors, two training sessions are a required part of the program. Fall Training is held in October and the Winter Workshop sessions are in February. You will be informed of the specifics as the dates approach. Please plan to attend.
THRIVE is a non-profit organization and would, therefore, greatly appreciate a contribution of $25.00 to help offset the expenses of record checks and training for your CAP application.(Upon submission of this application, you will be redirected to our donations page where you can choose to contribute to Thrive).
